Visit the TechZone October 12-17 to apply for your passport. No appointment needed! It’s first-come, first-served, and wait times may vary. Call 330.643.9145 to find out what you need to bring.

The last appointments are at 6 pm Monday-Thursday and 4 pm on Friday and Saturday. Walk-in passport services are not available on Sunday. Call with questions at 330.643.9145.

What you need to bring for a successful visit:

If you are applying for an adult passport:

A completed, unsigned passport application (Form DS-11) filled out in black ink.
Form DS-11 is available on the U.S. Department of State website.
Proof of U.S. Citizenship (one of the following must be submitted with the application):
Certified U.S. Birth Certificate
Previous U.S. Passport (may be expired, must be undamaged)
Consular Report of Birth Abroad Certificate of Naturalization/Citizenship
Proof of Identity
Valid Driver's License (plus second ID if issued outside of Ohio)
Undamaged U.S. Passport (if issued less than 15 years ago)
Valid government ID (city, state, or federal)
Valid Military ID
A Passport photo (available at the Library during your appointment for a $12 fee plus sales tax)
A check or money order that goes to the U.S. Department of State
If you are applying for a child 15 or under:

A completed, unsigned passport application (Form DS-11) filled out in black ink.
Proof of U.S. Citizenship (one of the following must be submitted with the application):
Certified U.S. Birth Certificate
Previous U.S. Passport (may be expired, must be undamaged)
Proof of Parental Identity for anyone listed on the child's birth certificate, such as:
Valid Driver's License (plus second ID if issued outside of Ohio)
Undamaged U.S. Passport (if issued less than 15 years ago)
Valid government ID (city, state, or federal)
Valid Military ID
A check or money order that goes to the U.S. Department of State
